Your First Visit

Welcome! We understand your first visit can be a big step, so we’re happy you’ve made the choice to come see us. This list will help you prepare for your first visit.

What should I bring?

A photo ID, a list of current medications (including supplements or vitamins), and — if you have insurance — an insurance card.

We encourage you to arrive 30 minutes early to check in and fill out any paperwork. Add time for parking, as well. You can find directions to all our locations here.

What should I do first?

When you arrive, check in at the front desk. Our staff will let you know what forms you will need to fill out and will be able to assist you with any questions or concerns. You can get a head start by filling out this Release of Information form.

After the front desk staff have checked you in, a medical assistant (MA) will call your name. They will review your medical history, ask you some questions about your health, and check your vitals such as your blood pressure and temperature.

What about the appointment itself?

Your first visit will likely be one where you will be doing a lot of sharing: talking about your medical history, the medications you take, and what your health goals are.

Several of our providers are bilingual. We also offer in-demand interpretation service using a tablet for most languages spoken in the US. Contact us in advance if you would like an in-person ASL interpreter for your appointment.
